Please select the type of device you're bringing in, if its not listed choose Other

If your device doesn't have a model or serial number just enter a brief description of what it is - i.e. Laptop with a smiley face sticker

Please bring the AC Adapter (if its a laptop) and keep hold of any bags. We don't need any discs or peripherals (unless you have an issue with them to)

Whats wrong or needs doing? (Even if you've spoken to us - please fill this in with as much detail as possible)

Does your computer require a password to login? If so please either jot it on a post-it note and place between keyboard and screen or change it to something simple like "password" or "1234"
